SERVICES
RENEWAL OF
EXPIRING PASSPORTS
If
your American passport is about to expire, you may apply for a new
passport at the Embassy's American Citizens Services Se
ction. For
renewal of a passport you must have:
- Your current passport;
- Other useful form of identification include a copy of previous
passport, a Consular Report of Birth Abroad , naturalization
certificate or any other U.S. passport issued to you in the past,
a driver’s license, military ID, and other government issued ID
cards.
- 2 recent identical photos 2" by 2" in size (5 cm x 5 cm), full
face - NOT profile; Pictures must have been taken within the last
6 months, with an image size from bottom of chin to top of head of
between 1 and 1-3/8 inches (no smaller than the dotted line and no
larger than the outer continuous line). Photographs must be clear,
front view, full face, taken in normal street attire without a hat
or dark glasses, printed on thin paper with a plain light (white
or off-white) background.
- A completed application for a U.S. passport (DS-82) by mail.
The form must be signed and completed in black ink.
- Fee for passport issuance is USD75 (for adults replacing
expired passports, and only when they can submit them) or USD 85 for minor
children under 16. Fees may be paid in dollars or
Rupiah at the embassy at the time of the application.

In 2002, the State Department began
issuing photo-digitized passports. The new passports are more
secure and tamper resistant, and are a major step in improving
passport security.
Photo-digitized passports can only be issued in the
United
States
. All non-emergency
U.S.
passports issued abroad take about 7-10
working days to be processed.

REPLACEMENT OF LOST OR STOLEN PASSPORTS
If your passport has
been lost or stolen, you may apply for a new passport at the
Embassy's Consular section in person. Documents required for
applicants over the age of 16 are:
- Proof of American citizenship. This may include U.S. issued
birth certificate, Consular Reports of Birth Abroad,
naturalization certificate, or any other US passport issued to you
in the past; as well as proof of identity such as a driver's
license, military ID card and other government issued ID cards;
- a police statement of the circumstances concerning the
loss/theft;
- a completed application for a U.S. passport (DS-11). The form
must be completed in black ink, but should not be signed until it
is processed by the Embassy
- a completed Statement Regarding a Lost or Stolen Passport
(DS-64). This form must be completed in black pen.
- 2 recent identical photos 2" by 2" in size (5 cm x 5 cm), full
face - NOT profile; Pictures must have been taken within the last
6 months, with an image size from bottom of chin to top of head of
between 1 and 1-3/8 inches (no smaller than the dotted line and no

- Fees for passport issuance are USD85 for
children under 16 years old, and USD 100.00 for adults - first
ten-year passport and in lost/stolen passport cases. Fees may be paid in dollars
or Rupiah at the embassy at the time of the application.
EMERGENCY
PASSPORTS
A U.S. Passport may be issued on a case by case
basis when a genuine emergency exist. Please discuss your concerns
with a Consular Officer directly.
PASSPORT APPLICATIONS
FOR CHILDREN UNDER 16
A person applying for
a U.S. passport for a child under 16 must demonstrate that both
parents consent to the issuance of a passport to the child usually
by both parents being present to sign the child's application or
that the applying parent has sole authority to obtain the passport. Exceptions to this
requirement are rarely granted but may be made in special family
circumstances or exigent circumstances necessitating the immediate
travel of the child.
When only one parent has appeared at
the Embassy or Consulate to apply for the minor's passport, we
require a written, notarized statement of consent from the other
parent.
The statement must be in English,
must give the non-appearing parent's unequivocal and unconditional
consent to the issuance of the passport, and must be notarized
before foreign notaries in most countries or at a U.S. Embassy or
Consulate abroad.
The purpose of the new requirement
that both parents consent be demonstrated is to lessen the
possibility that an U.S. passport might be used in the course of
international parental child abduction.
In all cases the child must be
present when the passport application is
submitted.
To apply passports for your children
under 16, please submit the following to the American Services Unit
at the Embassy:

The current or expired 5 year validity passport. You may also
be asked for additional photo identification.

A completed Application for a U.S. Passport (DS-11). The form
must be completed in black ink, but should not be signed until it
is processed by the Embassy.

Applicants under 16 must present their
original birth certificate/ Consular Report of Birth (CRBA) listing
both parents’ names. (A certified copy can also be
accepted).

Two recent identical photos 2" by 2" in size (5 cm x 5 cm),
full face - NOT profile; Pictures must have been taken within the
last 6 months, with an image size from bottom of chin to top of
head of between 1 and 1-3/8 inches (no smaller than the dotted
line and no larger than the outer continuous line). Photographs
must be clear, front view, full face, taken in normal street
attire without a hat or dark glasses, printed on thin paper with a

The passport fee of USD85.00 or the equivalent in Indonesian
Rupiah. The fee must be paid in cash.

In some cases, additional documents may be required. For
example if the child looks substantially different from their last
passport photo, please bring photos taken over the years so that
we may confirm the applicant’s identity.

Parents having concerns about passport
applications for any minor child who may become the victim of an
international parental child abduction, including children between
the ages of 16 and 18 who are not covered by this new law, should
contact the Office of Children's Issues in the Bureau of Consular

REPLACING A MUTILATED OR DAMAGED PASSPORT
If your passport has been mutilated or damaged,
you may apply for a replacement. All replacement passport applicants
must appear in person. Applicants under 16 are subject to all
conditions listed in the section "Passport Applications for Children
under 16 above.
To apply for a
replacement passport, please submit the following to the American
Services Unit at the Embassy:

A completed Application for a U.S. Passport (DS-11). The form
must be completed in black ink, but should not be signed until it
is processed by the Embassy.

An affidavit explaining the circumstances of the damage to the
passport. Blank affidavit forms are available at the Embassy. You
will be asked to take an oath and to sign in the presence of an
officer.

Two recent identical photos 2" by 2" in size (5 cm x 5 cm),
full face - NOT profile; Pictures must have been taken within the
last 6 months, with an image size from bottom of chin to top of
head of between 1 and 1-3/8 inches (no smaller than the dotted
line and no larger than the outer continuous line). Photographs
must be clear, front view, full face, taken in normal street
attire without a hat or dark glasses, printed on thin paper with a
plain light (white or off-white) background.

Depending upon how badly the passport is damaged, an applicant may be asked to
provide proof of American citizenship and identity. Examples of
proof of citizenship include U.S.-issued birth certificates,
Consular Reports of Birth Abroad, Certificates of Citizenship,
prior U.S. passports, or Certificates of Naturalization. Examples
of identity documents include drivers' licenses and military ID
cards. Identity documents must include names and photographs.

Applicants under 16 must have original or certified copy of
their birth certificate listing both parents' names.

The fee for replacement passports for applicants under 16 is
USD85.00 or the equivalent in Indonesian Rupiah. The fee must be
paid in cash. The passport is normally valid for 5 years. The fee
for replacement passports for applicants 16 and over is USD100.00 or
the equivalent in Indonesian Rupiah. The fee must be paid in cash.
The passport is normally valid for 10 years.
AN ADULT'S FIRST PASSPORT
AND ESTABLISHING PROOF OF USCITIZENSHIP
If you are a
first-time adult passport applicant or have not previously
established proof of United States citizenship, the documentary
requirements vary from case to case and will always require a
personal appearance with a consular officer.
PASSPORT STATUS
After you have
submitted a passport application, you can check the status of that
application by visiting this website: http://travel.state.gov/passport/get/status/status_2567.html.
You will be required to enter your last name, date of birth, and
social security number.
ADDITIONAL
PAGES
The Embassy will add
pages to a full passport free of charge. Americans must bring their
passports with them and fill out an application form. Pages can generally be added
in the same day.
To obtain additional
visa pages, you must complete an Application for Additional Visa
Pages (DS-4085: Amendment/Validation Application).
Please complete the form
using black ink and submit it with your passport to the American
Citizen Services section at the U.S. Embassy. U.S. passport holders 16 and
older must personally sign the DS 4085 but need not come to the
Embassy personally to request new
pages.
For children under 16, one parent needs to come with his/her passport and sign the
DS-4085 at the Consular Section of the Embassy. On that form, the parent should indicate his/her relationship next to the signature on the form. Though not required, the consul may ask to see an original or certified copy of the
child’s birth certificate showing both parents names. The child does not have to appear at the Embassy.
