Required Documents for Interview
An immigrant visa (IV or green card) is required for foreigners who will be in the U.S for an extended or indefinite period.
U.S. law defines IVs in five categories:
- Fiancé(e) / K visa
- Immediate Relatives - IR / CR visa
- Family Based visa(s) - F visa
- Employment-Based visa(s) – E visa
- Diversity Visa (DV) or green card lottery
Immigration is a two-step process. The first step requires the foreigner to be the beneficiary of an approved immigrant petition (I-130, I-140, I-360 or I-129F) from an eligible U.S. citizen relative or employer.

All Immigrant Visa (IV) applicants are seen by appointment only. Before coming to your IV appointment, please have in hand all required documents. If you do not bring them to your IV appointment you will be found ineligible for an IV interview. Only IV applicants with an approved immigrant (I-130/I-140) or fiancé (I-129F) or other approved petition may have an IV appointment.

Each applicant, including a family member of applicant, must have a separate documents. We must charge USD 1 per photocopy if you bring insufficient copies. All civil records must bear the original seal, stamp or signature of the official custodian of that record. Applicant can download all immigrant visa blank forms at Visa Application Forms.

All foreign language document(s) must be accompanied by a
complete and accurate English translation done by any translator competent in
English and the foreign language.
Document(s) must be signed and dated by the translator underneath this
statement: “This is a complete and
accurate translation into English of the attached document”. Notarization of the document(s) is
unnecessary.
Documents required for Fiancé(e) / K visa:
- Passport must be valid for 8 months or more
- Two copies of Electronic Visa Application Form DS-156 (EVAF).
- DS-156K and DS-230
- Birth Certificate
- Name change Certificate (if applicable from Petitioner or Beneficiary)
- Death or Divorce Decree (if applicable from Petitioner or Beneficiary)
- Military discharge, arrest, prison, court records; U.S. immigration documents (if applicable from Petitioner or Beneficiary)
- Police Certificate
Each applicant aged 16 or older must have police clearance from any country where the applicant resided for one year or more.

Please note Indonesian Police Certificate is not required - Original Affidavit of Support (Form I-134) from
the Petitioner along with supporting evidence:
a. One year U.S. income tax returns 1040 and W-2 (copies are acceptable)
b. Bank statements or proof of assets
c. Letter of employment or job offer - Two identical passport size photographs of each applicant
- Medical Examination result – please create a link to medical examination instruction
- Evidence of relationships (photos, cards, letters, emails, etc)

For each
document, the applicant must bring:
- Originals or original certified copies by the issuing office
- Photocopies of the original documents
- All documents should be accompanied by English translations
Documents required for IR/CR (spouse/child/parent(s) of U.S. Citizen) visa:
- Passport must be valid for 8 months or more
- DS-230
- Birth Certificate
- Name change Certificate (if applicable from Petitioner or Beneficiary)
- Marriage Certificate
- Death or Divorce Decree (if applicable from Petitioner or Beneficiary)
- Military discharge, arrest, prison, court records; U.S. immigration documents (if applicable from Petitioner or Beneficiary)
- Police Certificate
Each applicant aged 16 or older must have police clearance from any country where the applicant resided for one year or more.

Documents required for Family Based Visa:
- Passport must be valid for 8 months or more
- DS-230
- Birth Certificate
- Name change Certificate (if applicable from Petitioner or Beneficiary)
- Marriage Certificate
- Death or Divorce Decree (if applicable from Petitioner or Beneficiary)
- Military discharge, arrest, prison, court records; U.S. immigration documents (if applicable from Petitioner or Beneficiary)
- Police Certificate

- Death or Divorce Decree (if applicable from Petitioner or Beneficiary)
- Military discharge, arrest, prison, court records; U.S. immigration documents (if applicable from Petitioner or Beneficiary)
- Police Certificate

- Letter of employment or job offer

- Medical Examination result
- Non Refundable fee of USD 819.00, paid in IDR at current exchange rates. Payment must be made on the interview date with the Consular cashier.
For each document, the applicant must bring:
- Originals or original certified copies by the issuing office
- Photocopies of the original documents
- All documents should be accompanied by English translations
